4.      HCFA 1500–(Health Care Finance Administration) The Billing Form Explained  (The HCFA 1500 has changed to the CMS 1500 which is quite similar but with a few new things - see note above)

The HCFAA 1500 is a standard billing form that can be purchased at an office supply store.  Find out from each company that you are working with whether or not they will accept a copy or need a new form for each bill.  If you are billing by hand this may provide a shortcut to billing.  You can put all the patient information on the top half and make a copy of it.  You can then just fill in the bottom with the dates of service etc. and save the time of writing out the full bill.

Some insurance companies will not allow hand written bills and require a new form for each client.

There are also many computer software programs that will print on a HCFAA form or you can design a form using a word processing program that will allow you to complete the form on your computer.

Forms can also be handwritten. (I used to say that handwritten forms really didn’t matter, but they are starting to.)  I also have a HCFA 1500 template you can use and save to your computer.  You can download from my website – www.thebodyworker.com
