McIntosh: Highly recommended.  One of the first people to start talking about the nuts and bolts of practice building which are setting boundaries and framework.  Building a practice is based on ethics.  Ethics is just really about relationships.

Marketing Massage  by Monica Roseberry This is one of the best books on marketing your business that you can get.  The author actually visits over 1000 massage schools and interviews massage therapists and students and reports on what makes for a successful massage therapist. The number one thing that massage therapists had in common was the need to serve others.  This is really the heart of the intentions of my writing- to discover what it is really to serve and be of service.  

Business Mastery : A Guide for Creating a Fulfilling, Thriving Business and Keeping It Successful by Cherie Sohnen-Moe ISBN: 0962126543; 3rd edition 1997) Read an excerpt online and see table of contents. A massage therapists guide to setting up business and creating the practice that you want. Includes sections on business planning, starting a business, therapeutic relationships, marketing, and financial planning. Great for any stage in building a practice. Attracting Perfect Customers: The Power of Strategic Synchronicity by Stacey Hall, Jan Brogniez.  Basically explores the same message as I do in my website www.massagepracticebuilder.com and here.  A successful business is based on finding our ideal or "perfect customer" who is in tune with our personal and professional values and missions.  There are some good articles on the website at www.perfectcustomer.com  Also offers more support in the form of vision circles, classes and programs.
